Grounded synthesis for “Rapamycin & mTOR ovarian reserve”

In Ovarian Reserve and Primordial Follicle Activation [1], In Vitro Activation (IVA) with Mechanical Fragmentation carries a "Trial only" verdict (Prospective case series of 27-37 POI patients): Promoted follicle growth and successfully yielded oocyte retrieval, successful pregnancies, and live births — Shows clinical potential for select POI/DOR patients, but evidence is restricted to small, non-randomized case series. In Ovarian Reserve and Primordial Follicle Activation [1], mTOR Pathway Inhibition (Everolimus) carries a "Lab only" verdict (Xenotransplantation model studies (2026)): Successfully preserved dormant follicle pool and enhanced graft longevity — Valuable for tissue longevity and chemotherapy protection, though human clinical trial data for preservation remains pending.

[T1 (debated)] In a meta-analysis of seven randomized controlled trials involving 422 women with poor ovarian reserve or ovarian insufficiency, intraovarian autologous platelet-rich plasma (PRP) significantly improved surrogate reserve biomarkers (antral follicle count MD 0.81, AMH SMD 0.91, FSH MD -5.72 mIU/mL) but failed to significantly improve pregnancy rates compared to controls. [2] [3] [T1] Mammalian target of rapamycin (mTOR) signaling regulates primordial follicle activation, and unchecked hyperactivation of this pathway accelerates exhaustion of the dormant ovarian reserve. [4] [5] [T1] Coenzyme Q10 pretreatment significantly improves clinical pregnancy rates and oocyte yield while reducing cycle cancellation and miscarriage rates in women with diminished ovarian reserve undergoing IVF/ICSI, based on a meta-analysis of 1529 participants. [6] [7]

Calibrated baseline benchmark in Primordial Follicle Activation and Ovarian Tissue Engineering [8]: 20.0%–30.0% — Proportion of women with premature ovarian insufficiency possessing residual microscopic primordial follicles upon ovarian biopsy (Huber et al., 2021).
